2024 Hockey Australia National Registration Terms and Conditions
By applying to register to participate in competitions, activities or events of an Australian Hockey Association (AHO), including Hockey Australia Limited (HA), one of its Member Associations, regional associations or affiliated clubs, you (and, if you are under 18, you and your parent or legal guardian whose name and details appear in the requisite fields) acknowledge and agree that your application for membership will be considered and may be accepted or rejected in accordance with the relevant AHO's governing rules. If the AHO accepts your application, you (and, if you are under 18, you and your parent or legal guardian whose name and details appear in the requisite fields) acknowledge and agree that:
- You will be bound by and comply with all rules, regulations, by-laws and policies of the AHO, including the AHO's constitution and the National Integrity Framework (NIF), as amended from time to time (Rules) and directions, determinations and decisions made by the AHO and by these registration terms and conditions. The Rules and the NIF are made available from HA’s website at www.hockey.org.au/integrity or from the website of the relevant AHO.
- Without limiting the application of the Australian Hockey Code of Conduct or any right to terminate your membership under the AHO's governing rules, the AHO may terminate your membership registration at any time, including if your participation or behaviour interferes with any other participant's safety or enjoyment of hockey, subject to any requirements of the AHO's constitution.
- The information provided by you is current and correct at the time of application and will be updated by you as required within one month of any change.
- The personal information that the AHO collects from you may be used and disclosed for the purposes set out in the Australian Hockey Privacy Policy available from HA’s website at www.hockey.org.au/integrity and the privacy policy of the AHO, if any, including administering and marketing competitions, activities and other events; and the provision of hockey-related information and communication from AHOs. You may opt out of your email being used for communication purposes by contacting the AHO and/or managing your communication preferences online.
- The AHO may use third parties (such as IT providers, venue operators or survey providers) to collect your personal information or give them your information to help run hockey, as required by law. The AHO will always act in accordance with the Australian Hockey Privacy Policy and its privacy policy, if any.
- You accept all risks involved in training, competing and otherwise participating in activities related to or forming part of your membership of the AHO and your registration to participate in the sport, including the risk of transmission of viral illness, including COVID-19 and you have considered that risk, including any vulnerability that you may have. You warrant that you do not have a known pre-existing medical or other condition that makes it unsafe to participate in hockey competitions, activities and other events organised, authorised or recognised by an AHO (Hockey Activities) and agree, if reasonably requested by HA or any AHO of which you are a member, to provide a certificate from a duly qualified medical practitioner certifying you are fit to participate in Hockey Activities.
- You consent to receive any medical treatment that an AHO reasonably considers necessary or desirable for you during your participation in Hockey Activities if you cannot make a rational decision about your consent to any medical treatment that you may require. You also agree to reimburse the relevant AHO for any costs or expenses incurred in providing you with such medical treatment.
- You become entitled to the level of insurance protection as detailed at www.hockey.org.au/play/registration-and-insurance. This insurance level is not intended to be comprehensive coverage, and it is recommended that you investigate applicable insurance needs and coverage in relation to participating in Hockey Activities.
- Subject to any requirements of the AHO's constitution, you must notify the AHO in writing if you wish to cancel your registration. Following the AHO's confirmation to you of your registration cancellation, you will have no entitlement to the benefits that attach to registered participants in Australian Hockey. Refunds will only be provided in the circumstances set out in the Hockey Australia Refund Policy.
- Subject to applicable Rules (including the Australian Hockey Privacy Policy), you consent to photographs and video captured, recorded and live-streamed during your participation in any Hockey Activities and to AHOs using your name, image and likeness in any form or medium for:
- general marketing and promotional activities related to hockey in Australia; or
- the administration of hockey in Australia (e.g., to assist in identifying and investigating prohibited conduct under the NIF or for use in disciplinary proceedings, including tribunal hearings),
in each case without your further consent being necessary.
- If you do not wish to consent to your name, image and likeness being used for general marketing and promotional activities related to hockey in Australia, please notify the relevant AHO.
- An AHO may apply additional terms and conditions in relation to your registration, including, but not limited to, the collection of your personal information.
- Where there is any overlap between these terms and conditions and additional terms applied by an AHO in accordance with clause 10, the additional terms prevail to the extent of the inconsistency.
Australian Hockey Code of Conduct
Each AHO is committed to promoting and strengthening the positive image of hockey and its participants in Australia and to ensuring that everyone involved with hockey is treated with respect and dignity.
The Australian Hockey Code of Conduct, available from HA’s website at www.hockey.org.au/integrity or from the website of the relevant AHO, aims to ensure that everyone involved in hockey is aware of the standards of behaviour expected of them and the mechanism for dealing with any conduct alleged to breach the Code of Conduct.
Acknowledgement of this Code of Conduct is a requirement of your registration.
Safe Hockey
Each AHO is committed to ensuring the safety and wellbeing of all children and young people involved in our sport.
Your registration is subject to the terms of the NIF Children and Young People Safeguarding Policy, including the child-safe practices and recruitment and screening requirements contained therein.

If you see, hear or experience any suspicious or unwelcomed activity, including any misconduct relating to child safety, bullying, cheating, doping, gambling or match-fixing, report it confidentially to the Hockey Australia Integrity Unit or the Hockey Integrity Hotline.

Parent/Guardian Declaration
If the applicant for membership has not yet reached the age of 18 at the time of making this application or requires a guardian, I, the parent or guardian of the applicant, authorise and consent to the applicant undertaking the Hockey Activities. In consideration of the applicant's membership to the AHO being accepted, I expressly agree to be responsible for the applicant's behaviour and agree to accept, in my capacity as parent or guardian, the terms set out in this membership declaration. In addition, I agree to be bound by and to comply with the AHO constitution and all other rules, regulations, by-laws and policies of the AHO.

Hockey Queensland Member Registration Terms & Conditions
Registration Disclaimer: By registering as a member of Hockey Queensland, you agree to comply with the rules, constitution, regulations and by-laws, codes of conduct and member protection policy of Hockey Australia, Hockey Queensland, your Association and your Club.
Current registered players and non-playing officials will be covered by the Sports Injury Insurance Policy provided by Hockey Queensland’s agent Honan’s Insurance. In consideration of your application or registration being accepted, you acknowledge and agree that:
- Release and Indemnity: On consideration of the Association accepting your application for registration, you, to the extent permitted by law, release and will release the Association from all claims that you may have or may have had, but for this release arising as a result of, or in connection with, your registration and/or participation in any Hockey activities;
- You will be bound by and agree to comply with the constitutions, regulations and policies of Hockey Queensland, the Association and Club;
- You acknowledge that you are exposed to certain risks and that accidents can happen, which may result in you being injured or your property being damaged;
- You declare that you are medically and physically fit and able to participate in any Hockey activities. You will immediately notify the Club and Association of any change to your medical condition, fitness or ability to participate.
Hockey Queensland Privacy Statement: Hockey Queensland is committed to the protection of your personal information. Any personal information you provide to Hockey Queensland will be used for the purposes and related purposes of registration administration, registration statistics for research, developing and managing new and existing programs, for strategic and planning purposes, for the promotion of hockey in Queensland, for communicating and providing information to participants about their registration and/or their involvement in programs and competitions, including those of sponsors and other general hockey activities. Hockey Queensland will not disclose any personally identifiable information obtained from you to other parties or for purposes other than those stated above. It is the policy of Hockey Queensland to comply with the Privacy Act. Personal information about you is only obtained from information provided by you. This data is collected by Associations (and Clubs) affiliated with Hockey Queensland.
Registered players and non-playing officials can change or gain access to their personal information or advise their wishes for their personal information to not be used for any of the above purposes by contacting their Association/Club or by contacting Hockey Queensland.
Hockey Queensland Use of Image Statement: Hockey Queensland reserves the right to use at its discretion, any photographic material of you in any form of media, art, advertising, promotional material, merchandise or film coverage for the purposes of publicity and /or marketing, without any compensation to you or approval by you.

Hockey Queensland Refund Disclaimer: Hockey Queensland acknowledges there are certain circumstances that may warrant a refund of the registration fees. In this instance, submit a request for a refund with detailed reasons via email to hqoffice@hockeyqld.com.au. If the request is approved at Hockey Queensland’s discretion, the appropriate amount will be returned to the member. Please note that Hockey Australia’s policy differs from Hockey Queensland’s, any queries around this must be directed to them.
Hockey Queensland Dual Registration Disclaimer: Any player wishing to “Dual Register” for the current season, ie: play for two Clubs within two different Associations, must have met Hockey Queensland’s requirements for Dual Registration as follows:
- Player has completed registration online to their Primary Club for the current season and paid the relevant Hockey Queensland player registration fee at the time of registration;
- Player has requested permission from their Primary Club and Association, and obtained approval from them to dual register with a Secondary Club within a different Association for the current season;
- Player has advised the Secondary Club of their intention to dual register with them for the current season and provided the required documentation from the Primary Association;
- Final acceptance of dual registration is at the discretion of the Secondary Club based on the appropriate approval processes being undertaken with the Secondary Association.
In addition to the above requirements, a dual registered player must agree that the Primary Association has the right of first of refusal for selection in any representative teams, and subject to any agreement to the contrary, a dual registered player agrees that their first priority will be with their Primary Association and Club.
Nambour Blue Demons Hockey Club Inc
Nambour Blue Demons Hockey Club Inc. Fee & Code of Conduct Policy
When a player registers online with the Nambour Blue Demons Hockey Club Inc (the Club), they are agreeing with and will abide by the Club Fee Policy and the Club Code of Conduct. The Fee Policy is outlined below and the Code of Conduct is located on the website. The Club Treasurer (as all other committee positions) is a volunteer position. Please respect their valuable time by abiding with this policy. It is imperative that members communicate in a timely manner with the Treasurer any payment issues to avoid possible suspension from fixtures.
Fee Schedule
- The fee schedule for the forthcoming season will be discussed at the first management meeting of the club’s financial year.
- The final fee schedule for the forthcoming season will be agreed at a Club Management Meeting.
- The fee schedule is calculated based on an assumption of a 20-game season. If the SCHA fixtures for the season for that player are more or less than 20 games, there will be no adjustment to the fee schedule.
Fee Payment
- Payment can be made by cash, cheque or by direct deposit. It should be noted the club prefers a direct deposit into the club’s account.
- Bank Account: Nambour Blue Demons Hockey Club Inc. BSB: 064424, Account Number: 00907536, Reference: Player's name.
- Payment can be made in full, as per the published fee schedule, or in cases of financial hardship by a payment plan ( negotiated and agreed regular payments by direct debit) after a request in writing and agreement with the club’s treasurer at nbdhctreasurer@gmail.com
- Players may be suspended from playing if fees not paid.
- Hockey Australia (HA), Hockey Queensland (HQ), and Sunshine Coast Hockey Association (SCHA) registration payments are paid independently to Club Fees. Players will be required to pay the specified amount to both HA/HQ/SCHA and the Club. The Club Deposit must also be paid at the time on registration.
- If an eligible Junior player chooses to play only Senior games, full Senior Club Fees will apply.
- If a player is registered as a Junior, the players intent must be to play 6 games in a Junior Team to qualify for Junior Club Fees. This is to be 3 games played in a Junior Team by 31 May and further 3 games by the last fixture game. If this criterion is not meet, the player will be charged full Senior Club fees for the division they are playing in.
- In addition, any registered Junior player who is listed in a Senior Team during the season, they will be charged the Juniors Playing Seniors Levy.
Fee Discounts
- Discounts to the fee schedule can be applied for by contacting the Treasurer by email.
- Discounts can include but not limited to: A reduction for Queensland representative players, goal keepers who supply their own kit and senior players who are still at secondary school.
- The club accepts any current Queensland Government sports voucher discount scheme.
Fee Inclusions
- Club Fees include -
- Seniors – All Fixture and training costs, Club Fees, Senior Presentation, Umpire Levy, SCHA Fees billed to the Club.
- Juniors – All Fixture and training costs, Club Fees, Junior Presentation, Umpire Levy, SCHA Fees billed to the Club.
Fee Exclusions
- The Club’s fees do not include any Hockey Australia, Hockey Queensland or SCHA Player Registration Fees which are paid separately online during registration through the Registration Portal by players/parents and not to the Club.
Fee Non-Payment
- Players will not be able to take the field for training or playing if HA/HQ/SCHA registration is not completed as they will not be covered by insurance.
- Players also will not be able to play if Club Deposit is not paid.
- Players will not be able to play if Final Invoice is not received by the due date unless there is a prior agreed and signed payment plan (Treasurer and Player). If a player does not fulfil agreed Plan, they will not be able to play.
- Players will not be able to play in any final if fees have not been paid in full unless there is a prior agreed and signed payment plan (Treasurer and Player). If a player does not fulfil agreed Plan, they will not be able to play.
- Players will not be able to attend presentations if fees have not been paid in full unless there is a prior agreed and signed payment plan (Treasurer and Player).
- Players who remain unfinancial as of the 31st December (unless an Agreement is in place with the Treasurer) will be declared unfinancial to the SCHA and will remain unfinancial and unable to play for any Club, until the fees are paid in full.
Fee Refunds
- No player fees are transferable to another player or member.
- Refunds in fees can be applied for by contacting the Treasurer by email at - nbdhctreasurer@gmail.com
- The Nambour Deposit will not be refunded unless Player has not taken the field.
- Examples of cases where the management committee will consider a refund include but not limited to: pregnancy, injury, illness, forced relocation.
- Refunds in fees will be discussed at the first Club’s Management Meeting following the submission of the request to the Treasurer.
- Players will be notified of the outcome within 10 days of the Meeting. If a refund has been approved and bank details have been provided, a refund will be made within 14 days.
- All non-refundable fixed costs WILL NOT form part of the pro rata refund.
- If a player has played no games, then the player can apply for a refund. The player is to contact HQ direct for discussion of refund for all HQ Fees.
- Players who have successfully applied for a refund and who wish to attend the end of season presentation will have to pay an extra cost to attend.

Nambour Blue Demons Hockey Club Inc. Fee & Code of Conduct Policy (Draft 2025)
When a player registers online with the Nambour Blue Demons Hockey Club (the Club), they are agreeing with and will abide by the Club Fee Policy. - Payment plans can include the Club deposit if a request is made to the Treasurer prior to a player’s registration.
- Players may be suspended from playing if fees not paid.
- If an eligible junior player chooses to play only senior games, full senior Club Fees will apply.
- If a player is registered as a junior, the players intent must be to play 6 games in a junior Team to qualify for Junior Club Fees. Fee Inclusions
- Club Fees include -
- Seniors – All Fixture and training costs, Club running costs, Senior Presentation, Umpire Levy, SCHA Fees billed to the Club.
- Juniors – All Fixture and training costs, Club running costs, Junior Presentation, Umpire Levy, SCHA Fees billed to the Club.

- Examples of cases where the management committee will consider a refund include but not limited to: pregnancy, injury, illness, forced relocation.
- Refunds in fees will be discussed at the first Club’s Management Meeting following the submission of the request to the Treasurer. A minimum of 4 consecutive matches missed will have to have taken place for the refund to be considered. Refunds will be pro rata of a 20-game season minus the deposit payment.
- Players will be notified of the outcome within 10 days of the Meeting. If a refund has been approved and bank details have been provided, a refund will be made within 14 days.
- All non-refundable fixed costs WILL NOT form part of the pro rata refund.
- If a player has played no games, then the player can apply for a full refund from the Club/SCHA/HQ/HA minus any non-refundable processing fees.
- Players who have successfully applied for a refund and who wish to attend the end of season presentation will have to pay an extra cost to attend.
- Players who end the season in credit through over payment of fees or club payments, which include but not limited to: umpiring or club management duty, will be offered either a refund by bank transfer or a credit to apply against the following season fees.
Uniform Payments
- All payments for uniforms are to be made at time of purchase either through the club website or a Square payment. No invoices will be created.
- Players will be responsible for any processing costs which are non-refundable.
